What Is the Community Health Improvement Plan?
To inform the development of the Community Health Improvement Plan (CHIP), a Community Health Assessment (CHA) was conducted in 2017. The CHIP is a guide for implementing community-owned, public health initiatives throughout the next five years. The Partnership for a Healthier Fairfax published its first CHIP in 2013. Its next five-year plan, CHIP 2019-2023, is available. Due to COVID-19, the Partnership for a Healthier Fairfax extended the CHIP until 2024 to allow time for more work on the priority issues and the completion of the Community Health Assessment.
- Healthy Eating – promoting the accessibility, availability and affordability of healthy food for all
- Healthy Environment and Active Living – improving the community environment to support active living and good health for all, and
- Behavioral Health – promoting social and emotional wellness for all.
The CHIP describes the proposed goals, objectives, and key actions for each priority issue area. More detailed implementation plans will be developed outlining how the goals and objectives will be met, including specific activities, milestones, timeframes, key partners, and evaluation measures. As a community-owned plan, the ultimate success of this CHIP depends on the active support and commitment of those individuals and organizations that play a direct or indirect role in affecting lasting impacts on health outcomes across the Fairfax community.
Since the development of the CHIP, each priority issue area has made progress on many goals, objectives, and key actions. Below are the CHIP Annual Reports for each year.
