What is a Community Health Assessment?

A community health assessment captures health-related data and statistics about the community, health-related partners and assets in the community, and the experiences of community members. Data from the community health assessment informs decision-making, prioritization of health issues, and development of plans to improve the health of the community. 

The Fairfax County Health Department, in conjunction with the Partnership for a Healthier Fairfax, recently completed the 2023 Community Health Assessment. This assessment will inform the development of a new Community Health Improvement Plan (CHIP) in 2024.
